Social prescribing template
Referring patients to non-health support, especially for those facing complex social or legal issues, is a key part of holistic care. This template, to be completed and updated by practice staff, offers suggestions of key service types which Surgeries should be aware of in their local area. Some organisations which provide services nationally have already been suggested.
